Sudden Severe Colicky Abdominal Pain, Vomiting and Constipation

An 86-year-old woman complained of sudden onset severe colicky lower abdominal pain for 4 hours. This was associated with vomiting and constipation.

Physical examination showed that she was in agony with stable BP and pulse. The abdomen was distended, with marked local tenderness and guarding over the central lower abdomen. There was no physical sign of inguinal or femoral hernia. Laboratory investigations showed normal white cell count.
