A 40-year-old man, with good past health, complained of lethargy, polydipsia and polyuria for 3 months. Physical examination was unremarkable aside from muscle hypotonia and bone tenderness.
Laboratory investigations revealed hypercalcaemia and hypophosphataemia. Alkaline phosphatase and parathyroid hormone levels were elevated.
