A 23-year-old man presented to the neurology clinic with a 1-month history of double vision and eyelid drooping on the right side that worsened at the end of the day. He reported no slurred speech, difficulty swallowing, or limb weakness.
The neurologic examination was notable for restricted eye abduction and adduction on both sides and ptosis on the right side. Manual raising of the ptotic right eyelid resulted in drooping of the left eyelid.
