Interpretation

The ladder diagram depicts Atrial Flutter with 2:1 and 3:2 Wenckebach conduction.

The interesting aspect is the abnormal conduction with RBBB of the second transmitted impulse. The pause due to Wenckebach dropped beat, by the Ashman effect, sets the stage for the aberrant conduction.
