A 70-year-old male presented to the Emergency Department complaining of palpitations, fatigue, dizziness, epigastric, and chest pain. Further history revealed melena for one week with no history of significant alcohol consumption.
His past medical history is notable only for ischaemic heart disease requiring stents 10 years ago. His medications included aspirin, atorvastatin and metoprolol. His hemoglobin was 72 g/L (115 – 155).
