This post is an answer to the ECG Case 229
- ST elevation in II, III, ?aVF (difficult to see given baseline noise)
- ST depression in leads V2-4 and I, aVL
- T wave inversion in lead V2
- Subtle ST elevation in leads V5-6

Infero-postero-lateral OMI
What happened next?
The patient was discussed with cardiology team and underwent emergent angiogram, which showed:
- RCA – mild irregularity
- LAD – 30% stenosis
- OM2 – 99% occlusion –> stented
- LV function preserved
Uneventful recovery and discharged after 2 day in-patient stay.
